A Circle of Care provides in-home senior services with the principle goal of making each client’s day the best that it can be. We understand there are times when a little extra companionship or light housekeeping is all that is needed to keep you or a loved one safe, active and engaged. At other times, the aging journey may require short-term or long-term live-in 24-hour care. Our focus remains the same—to help make each day the best that it can be.
Our care begins with your first call. A care manager will arrange to meet with you and perhaps a family member to complete an in-home needs assessment. We listen carefully to your needs and preferences and then set goals and develop your unique personal plan of care. We will be there to provide care—and when desired—to facilitate coordination with your unique circle of care “team” which often consists of friends, family, caregivers, doctors, pharmacists, specialists, faith community, and other outside services. Your caregivers are closely managed, with frequent communication so that we know just how things are going. This provides critical information to anticipate and make timely care adjustments necessary to help keep you safer, healthier and happier.
